This is a dedication from Consalus Ponce de Leon to Pope Sixtus V, to whom Ponce de Leon was a "cubicularius" (chamberlain). "Pont. Opt. Max." (Pontifex Optimus Maximus) is a standard title for the Pope, who is the highest priest of the Catholic Church. It is dated "Kalendis Ianuariis" (January 15) 1587.
